We are looking for a dedicated and experienced Care Coordinator to join our team. The successful candidate will be responsible for assessing clients’ needs, developing individual care plans, coordinating various care services, and monitoring care delivery to ensure high-quality support. This role requires excellent organisational, communication, and interpersonal skills, as well as a compassionate approach to care.

  • Conduct initial assessments and regular reviews of clients’ care needs in collaboration with healthcare professionals and families.
  • Develop, implement, and update personalized care plans in partnership with clients and their families, ensuring their preferences and needs are met.
  • Coordinate and schedule the deployment of care staff to meet the requirements of clients’ care plans effectively.
  • Monitor the quality of care delivery, ensuring all care services comply with regulatory standards and best practices.
  • Act as the main point of contact for clients, families, care staff, and external healthcare providers, facilitating communication and resolving any issues.
  • Maintain accurate and up-to-date records of care activities, assessments, and communications in accordance with legal and organizational requirements.
  • Participate in staff training, development, and meetings to ensure a cohesive and competent care team.
  • Contribute to the continuous improvement of care services, identifying areas for enhancement and implementing changes as needed.
  • NVQ Level 3 in Health and Social Care or equivalent is essential. Higher qualifications in care management or related fields are desirable.
  • Proven experience in a care coordination or similar role within the health and social care sector.
  • Comprehensive knowledge of care standards, regulations, and best practices.
  • Ability to conduct assessments and develop detailed care plans.
  • Excellent communication and interpersonal skills, with the ability to work effectively with a wide range of stakeholders.
  • Strong organizational and problem-solving skills, with a capability to manage multiple priorities.
  • Compassionate and patient-focused approach.
  • Strong leadership and team management skills.
  • Proficient in using care management software and Microsoft Office Suite.
  • Ability to work under pressure and adapt to changing situations.
  • Office-based role with the requirement to travel to client sites as needed.
  • Standard working hours with the need for flexibility to meet the demands of the service, including occasional evenings and weekends.
